I am the social prescriber working for the Newcastle Outer West PCN. I will be based at Denton Park Medical Group. My role will be to work closely with the frailty team which includes a nurse and healthcare assistant as part of the PCN team. I will be receiving referrals from the GPs and other members of the PCN to help develop personalised community-based health outcomes for older people 50+ in Newcastle outer west. I will work with individuals to promote independence and increase the persons sense of emotional wellbeing. Helping them stay active for longer. Each plan will be tailored to their needs. For instance, it could be linking them into services and activities in the community that meet their individual needs. This will in time help build confidence, sustain links and friendship between other local people in the community.
