Susan Gibson

I am a Healthcare Assistant with the Frailty Team covering the six GP surgeries in the Outer West PCN. It is my role to work with and support, the Sister and Social Prescriber on the team to address the healthcare needs of frail, vulnerable and housebound patients unable to attend surgery. I conduct home visits for such things as repeat BP readings, blood samples and annual healthcare reviews. I will ensure those I visit receive the same health monitoring and care any other patient with the practice receives
