I am a Social Prescribing Link Worker for the Outer West PCN.

My role is to provide support to discuss and tackle issues may which affect your health and well-being.

To motive, empower and encourage you to take positive actions to address these issues and if appropriate link in with other healthcare professionals, and to signpost you were necessary to other groups and activities who can provide support.

I have worked in the social care profession for many years and look forward to utilising these skills in a different field.
