I am a Frailty Nurse working for Newcastle Outer West PCN, based at Denton Park Medical Group. As part of the newly formed Frailty Team, I am looking forward to improving care for frail housebound patients in Newcastle Outer West. With an initial focus on ensuring that annual reviews for long term conditions are up to date, the aim of the service will also be to complete multidimensional holistic assessments to identify patient needs and reduce the impact of frailty. Working alongside the Frailty Social Prescribing Link Worker and the wider multidisciplinary PCN team I aim to address factors contributing to frailty and to improve outcomes for housebound patients.
