Our Community Link Workers offer our Social Prescribing Service. They offer patients the time to look at ‘what matters to me’, time to explore issues and challenges and offer the opportunity for a person to make meaningful changes in their life. The aim is to empower and encourage patients to take control of their own mental health and wellbeing. The Community Link Workers will provide practical and emotional support, signpost to local services or groups, support a person in accessing appointments or services or groups. They will deliver brief interventions to educate someone to have the skills and tools to move towards independent living.
What is social prescribing?

Who is this service for?
- People aged 18 and over
- People struggling with emotional distress, wellbeing or lifestyle challenges, one or more long term conditions, social needs, loneliness or isolation.
- We also offer a Learning Disability – Social Prescribing Service
This service is not suitable for:
- People under 18 years of age.
How can I make an appointment?
Contact your GP Practice and ask for a referral to the PCN Wellbeing Team for the ‘Social Prescribing’ Service.
