Learning Disability – Social Prescribing

This is a Social Prescribing service for patients who have a learning disability. Social Prescribing means supporting you to find things to do in your local area that can help your physical health, mental health and wellbeing – aiming for you to feel happier and healthier.

Our Community Link Workers can support you with your Annual Health Check at your GP Practice, and to complete your goals in your Health Action Plan. You can also have support with routine appointments if you are feeling worried or have any concerns or questions.

They can support you to join groups, find voluntary opportunities, prepare for employment, meet new people in a safe way, and to feel less lonely.

Who is the Learning Disability Service for?

  • Patients with a Learning Disability
  • People aged 16 and over

This service is not suitable for:

  • People under 16 years old

How do I make an appointment?

Contact your GP Practice and ask for a referral to the PCN Community Link Worker (Learning Disability).
