I’m Becca, the Frailty Advanced Nurse Practitioner and Lead for our Frailty Team.

I support patients living with complex frail needs. In my role as an Advanced Practitioner, I see frail people living at home following referral from their GP to undertake a Comprehensive Geriatric Assessment (CGA) and manage their ongoing care on behalf of the GP. This holistic assessment looks at medical needs, mobility, cognition, medications, functional ability and social circumstances, enabling tailored, person-centred care planning.

I am also leading the development of our new Frailty Service, shaping how we deliver proactive, coordinated support and overseeing the running and continued development of the Frailty Team.
