I’m Lauren, one of the frailty nurses. This role is truly my passion, and one I’m proud to do. I support patients with complex frailty needs, helping them remain in their preferred place of care while effectively managing their needs. I carry out holistic assessments that consider medical conditions, mobility, cognition, medications, and social circumstances, enabling tailored, person-centred care planning.
