Hannah Patterson

I am the Outer West’s Primary Care Mental Health Practitioner with a background in mental health and social work. I am a positive, passionate, and people-focused mental health professional, and I care about proving a thoughtful, inclusive, and person-centred support. For a number of years, I have worked with adults above 18 years of age with complex mental health needs within Secondary Care. I offer appointments without patients needing to see the GP first. I can offer an assessment, brief evidence based therapeutic intervention and referrals to other appropriate services.
