This is a service that can support you if you are concerned about your weight and the impact this may be having on your health or wellbeing. Studies show that losing 5% of your weight can have significant health benefits such as; lowering risk of heart disease, Type 2 diabetes, stroke and high blood pressure. Further, it can also help with improved confidence and self-esteem.
Our PCN Dietitian can offer support and guidance through a one to one tailored nutrition therapy plan in relation to diet/lifestyle. This will be a one to one appointment; either face to face, telephone or video.
Who is the service for?
- Adults, aged 18 and over
- People with a BMI greater than 30kg/m2, or 27.5kg/m2 for black African, African-Caribbean, and Asian family origin
This service is not suitable for:
- People under 18 years old
- If you are currently pregnant
- If you have a diagnosed eating disorder e.g. Bulimia
- If you are already receiving care from the Newcastle Specialist Adult Weight Management Service
How do I make an appointment?
You can contact your GP Practice and ask for a referral to the PCN Dietitian. Any health care professional in the Practice can also refer you to the PCN Dietitian.
