What is Outer West PCN?

We are a group of GP Practices located in the west of Newcastle upon Tyne, brought together to form the Outer West Primary Care Network (PCN). This includes six GP Practices

The GP Practices in the Outer West of Newcastle have a history of working together, this was formalised when the PCN was created in 2019 in response to NHS England’s effort to focus on the needs of the local neighbourhood.

Whilst we are working together, each GP Practice retains its uniqueness, with patients still having access to appointments at their practice in the usual way.

What is our Vision?

“To be a collaborative needs-led Network that has provided significant and demonstrable improvement to the health of our population”

We are working to provide improved care and improved health and wellbeing, to reduce health inequalities and improve access to care in a proactive, coordinated way. In short, our PCN is about working with the community, for the community, in the community.

How do we do this?

To begin to make our vision a reality we have already recruited several new staff and launched additional services in the Outer West area. To learn about our patient’s needs, we worked with Healthwatch in early 2021 to run a survey to ask patients about the services they felt were needed. We were pleased to see that a number of the services mentioned, were services we had recently launched.

Our GP Practice teams are working closely together to set up these services at a local level, bringing together the GP Practices with local community groups, the voluntary sector, social care, pharmacy, mental health and hospitals in order to improve the services and care offered. The services are focused on the specific needs and demographics of the people in the geographical area.

Our GP Practices are sharing resources and expertise, recruiting staff to work across the PCN to enable us to have a bigger team, working flexibly.

How are we managed?

We have a management group where each of the Practices are represented through a GP and Practice Manager, this group meets monthly.

Our Clinical Director is Dr Ian Morgan. Ian has worked at Newburn Surgery for 5 years and continues to work as a GP whilst leading the PCN.
